Ankle Sprain, High Ankle Sprain, or Fracture? Knowing the Difference

Ankle injuries are among the most common reasons people end up in urgent care or the ER. But not all ankle injuries are the same—and the treatment can be very different depending on whether it’s a simple sprain, a high ankle sprain, or a fracture.

A standard ankle sprain happens when the ligaments on the outside of the ankle are stretched or torn, often from rolling your ankle. Most of these improve with time, rest, and physical therapy focused on balance and strengthening exercises. High ankle sprains, on the other hand, involve the syndesmosis—the ligaments that connect the two lower leg bones together above the ankle. These injuries are trickier, can take much longer to heal, and usually require specialized exercises to restore strength and stability before a safe return to sports or activity.

Ankle fractures are different altogether. Instead of ligaments, the bones themselves break, which can cause the ankle joint to shift out of place. While some stable fractures can be treated in a cast or boot, many require surgery to realign and stabilize the joint with plates and screws. Unlike sprains, fractures can’t be “walked off,” and delaying care can lead to long-term problems like arthritis or stiffness.
